Win7 - New User profiles are not working correctly, even after format/reinstall

We just put out a few machines last week and basically one user used it during that week. When a new user finally logged in, he had issues. We eventually gave up, formatted, reinstalled Windows with the Dell CD and manually installed the apps, all Windows updates, etc. Same problem, different user. PC is on a domain, no fancy setups or strange problems

When a new user (2nd+ user) logs in,
1.) Outlook gets the error “The name cannot be matched to a name in the address list.” Clicking OK, gives a popup prompt asks for the server and mailbox name and for some reason is pre-filled with “mail.omu.local” which is wrong, and username of “SYSTEM”. 
Before formatting, I could change this to the correct mail server address, and enter the user’s name to replace SYSTEM, and Outlook would load. Now I get, “The connection to Microsoft Exchange is unavailable. Outlook must be online or connected to complete this action.”

2.) Some random activities cause Windows Explorer to crash. Example, Control Panel and clicking on Credential Manager causes instant crash of Explorer.exe.

3.) We have an internal app does not work on this machine either (except under the first created profile). It launches and says “Application cannot be started, contact vendor for more information.” I can view details on this error which is in the screenshot. We've tried to directly run it from the server, which doesn't work either.

I have no idea where to start troubleshooting next.
